Individual Membership Fee: $1500.00
Family Plan: If both parents join Premier at $1500 each, their children
under age 18 are free!
The total membership fee is due upon execution of the membership agreement and after a face to face meeting of the prospective patient with their Premier Physician to ascertain mutual compatibility. The annual membership fee will be billed annually to the patient’s credit card number on the anniversary date to assure no lapse in Premier services. The annual membership fee is not in lieu of insurance but is for extra services provided to our members. The membership fee is not covered by insurance. We refer you to your tax advisor or human resource representative for clarification as to whether the membership fee qualifies for reimbursement from a Healthcare Savings Account or other tax-advantaged plan.
In addition to the annual membership fee, all members will continue to pay for medical services as they do in the current fashion. For example, our participating physicians will still bill insurance for office visits, hospital visits, consultations, lab fees and diagnostic fees and we will accept assigned payments. Our participating physicians will continue to accept most of their current insurance plans for coverage of primary care service. In addition, our participating physicians will continue to be a Medicare provider and accept new Medicare patients.
