How can you become a member and access all the benefits of Premier Patient Healthcare?

Step 1:

Please call to request an enrollment package. For mail requests, please include your name, current mailing address and phone number.

Step 2:

Complete and sign the Membership Agreement and mail or deliver it to our office. Unless you pay by check, we will charge your membership fee to your credit card or debit your checking account according to the terms outlined in the Membership Agreement. Please deliver the completed Membership Agreement to:
