Professional Services:

Amenities enhancing the delivery of the primary care services will include:

• 24/7 physician access via office phone during regular business hours, or via his cell phone after hours and on weekends, as well as access via e-mail or fax 
• Personal health care advocacy when you are seeking other health care services, i.e. offering clarification on subspecialist recommendations, interceding for you when there may be conflict. 
• Administrative pre-employment/sports/camp physicals, FMLA form completion, DMV permit facilitations, DME and prescription pre-authorizations provided at no charge. 
• Personalized medical research on specific issues, whether they are directly or obliquely related to you. 
• Home visits arranged, if medically necessary 
• Free consultations for cosmetic services including laser treatments (hair reduction, skin tightening, varicose vein ablation, acne reduction, brown spot ablation, fine wrinkle softening), and Botox and dermal fillers (e.g. Juvaderm) for wrinkle reduction.

• Maintenance of your Premier Patient Healthcare medical records in an electronic medical record format that is portable. 
• Access to a secure website to request appointments, prescription renewals or communicate with your physician and staff via e-mail. 
• Electronic newsletters covering topical and preventive healthcare issues periodically distributed. 
• Same day or next business day appointments for any medical reason 
• Unhurried, 30 minute minimum appointments 
• Support personnel dedicated to assist members

Advocate with Specialists

Our physicians will assist you in navigating the medical world outside the primary care office by communicating with 
outside specialists to optimize your care. This may include visiting you in a hospital setting though you may be on another physician’s service.

Hospitalization Management

When you become hospitalized, Premier participating physicians will work as your advocate in an effort to make your experience easier. At the present time, Dr. Montalbano admits only to Mount Vernon Hospital. If you are admitted to another facility, Dr. Montalbano will interface with the physicians at that hospital treating you to improve the coordination of care, communication with family members, as well as faciliate the transfer of medical records to assist in your care.

Personalized Research

Premier participating physicians will conduct individualized research on the personal medical issues concerning you and your family. After thoroughly researching your area of concern, your physician will report to you the findings, along with any specific recommendations.

Direct Access To Your Physician 24/7

In addition to the office phone number access during business hours, all Premier members will receive their physician’s personal cell phone number for after hours needs, effectively making him available 24 hours a day, 7 days a week. In addition, you may reach your physician by email or fax. Whether it is after hours and you simply forgot to get a prescription renewed or you are having breathing problems on the weekend, you will have the ability to reach your physician immediately. No more hassle wading through convoluted steps on an automated phone system and getting the on-call physician. In addition, our dedicated staff will respond promptly to emails and phone calls from patients during business hours. Premier’s business hours will be 8 am to 5 pm EST Monday through Friday.

Same Or Next Day Office Visits

Premier understands that your time is important and that the need may arise for you to see your physician on the spur of the moment. Premier’s participating physicians will make every possible effort to see you on the same day you call, for any medical reason. No longer will you have to wait to see your physician or need to schedule your annual exam months in advance. 

Unhurried Appointments With Minimal Wait Time

We recognize that your time is extremely valuable and we will attempt to schedule appointments appropriately so that your time is not wasted waiting. We will work hard to make sure that our members will have minimal to no wait time in the reception area. In addition, by limiting membership, Premier participating physicians can schedule as much time as necessary to meet your needs at each visit. We schedule appointments in 30 minute slots, not the standard 12-15 minute slots routinely used in primary care practices. If more time is needed in future appointments, this can be arranged.

Personal Electronic Health Record

Premier participating physicians will have the ability to review your vital medical information via a secure website. With your permission to release medical information, an electronic medical format can facilitate communication of your critical medical history to those other care providers when you are out of the geographic area and need of health care services, or if you have relocated.

Secure website Access to Communicate Health Questions

Premier members will have access to a secure website where they will be able to communicate via e-mail their specific health questions to their physician, request an appointment or request a prescription refill. Premier will walk you through this process of registering once you become a member.
