Premier will contract with Springfield Family Medicine for the following Professional Services:
• An Annual Premier Personal Health Assessment usually lasting 1-2 hours documenting or updating surgical and hospital events, pertinent family history, advance directive status, exercise habits, social behaviors, immunizations,and interim changes in the review of all major health systems. Optionally a physical fitness evaluation conducted by a certified athletic trainer and a one-on-one lifestyle nutritional consultation for adult members can be arranged on a fee for service basis.
• Your Premier physician will act as your advocate when you are seeing other health care services, i.e. offering clarification on recommendations, interceding for you when there may be conflict.
• Administrative and sports physicals and disability evaluations provided at no charge.
• Your physician will conduct personalized research on your medical concerns.
• Home visits arranged, if medically necessary
• Maintenance of your Premier Patient Healthcare medical records in an electronic medical record format that is portable.
• Access to a secure website to request appointments, prescriptions or ask medical questions via e-mail.
• Electronic newsletters covering topical and preventive healthcare issues periodically distributed.
• Free consultations for cosmetic services including laser treatments (hair reduction, skin tightening, varicose vein ablation, acne reduction, brown spot ablation, fine wrinkle softening), and Botox and dermal fillers (e.g. Juvaderm) for wrinkle reduction.
In addition to the above Professional Services, Premier will offer its members the following conveniences at no charge:
• 24/7 physician access via office phone during regular business hours, or via cell phone after hours and on weekends, as well as access via e-mail or fax
• Same day or next business day appointments for any medical reason

• Support personnel dedicated to assist members
• Private waiting area with access to phone and refreshment
• 15% discount off our professional services is given for Premier Patients who contract for these cosmetic treatments as well as for our in house MediFast physician managed weight loss program

Premier Personal Health Assessment
Modeled after successful “executive physicals,” Premier Patient Healthcare participating physicians will customize an age appropriate, personal health assessment for you. This is a more comprehensive history and physical than is experienced in a typical history and physical exam. An exercise stress test and pulmonary function test to determine your aerobic capacity is usually included as is hearing, vision, and pulse oximetry screening. Optionally, adult Premier Patient Healthcare members can be scheduled for a one-on-one lifestyle dietitian consult as well as a personal fitness evaluation conducted by a certified athletic trainer to assess strength, flexibility and stamina with goals to be monitored throughout the year. While these elective services are fee for service, they complement a patient’s Annual Health Assessment. Recommendations and a summary of your Premier Personal Health Assessment will be provided to each member in a written or electronic format, with understandable actionable goals that can be compared year to year.
Our physicians will assist you in navigating the medical world outside the primary care office by communicating with
outside specialists to optimize your care. This may include visiting you in a hospital setting though you may be on another physician’s service.
When you become hospitalized, Premier participating physicians will work as your advocate in an effort to make your experience easier. At the present time, Dr. Montalbano admits only to Mount Vernon Hospital. If you are admitted to another facility, Dr. Montalbano will keep in touch with the physicians at that hospital who are treating you to improve the coordination of care and communication with family members.
Premier participating physicians will conduct individualized research on the personal medical issues concerning you and your family. After thoroughly researching your area of concern, your physician will report to you the findings, along with any
specific recommendations.
In addition to the office phone number access during business hours, all Premier members will receive their physician’s personal cell phone number for after hours needs, effectively making him available 24 hours a day, 7 days a week. In addition, you may reach your physician by email or fax. Whether it is after hours and you simply forgot to get a prescription renewed or you are having breathing problems on the weekend, you will have the ability to reach your physician immediately. No more hassle wading through convoluted steps on an automated phone system and getting the on-call physician. In addition, our dedicated staff will respond promptly to emails and phone calls from patients during business hours. Premier’s business hours will be 8 am to 5 pm EST Monday through Friday.
Premier understands that your time is important and that the need may arise for you to see your physician on the spur of the moment. Premier’s participating physicians will make every possible effort to see you on the same day you call, for any medical reason. No longer will you have to wait to see your physician or need to schedule your annual exam months in advance.
We recognize that your time is extremely valuable and we will attempt to schedule appointments appropriately so that your time is not wasted waiting. We will work hard to make sure that our members will have minimal to no wait time in the reception area. In addition, by limiting membership, Premier participating physicians can schedule as much time as necessary to meet your needs at each visit. We schedule appointments in 30 minute slots, not the standard 12-15 minute slots routinely used in primary care practices. If more time is needed in future appointments, this can be arranged.
Premier participating physicians will have the ability to review your vital medical information via a secure website. With your permission to release medical information, an electronic medical format can facilitate communication of your critical medical history to those other care providers when you are out of the geographic area and need of health care services, or if you have relocated. If requested, Premier can provide you with your electronic health record via CD or other medium.
Premier members will have access to a secure website where they will be able to communicate via e-mail their specific health questions to their physician, request an appointment or request a prescription refill. Premier will walk you through this process of registering once you become a member.
